Worn fork or hub assembly. Time for transmission work. Some standard transmissions don't need to come out just the back cover needs to come off and replace parts. Some need to come all the way out to replace the parts that are bad. Just don't remember which transmission you have. Transmission shop will let you know along with the price to repair
You can keep driving in 4th gear, you'll just use a bit more gasoline. I've seen frozen shift bushings at the end of the shift lever under the vehicle that can cause the transmission to pop out of gear. You might want to have the bushing replaced if the shift lever feels stiff.

- My 1999 manual honda prelude does not shift into 5th gear or reverse smoothly? My gears are smooth from 1-4 but then takes a lot of effort to go to 5th or reverse. Also when you get out of 5th or reverse it doesn't go straight into neutral like a normal manual car. They still work but it is very annoying that i have to push it into the gears and pull them out. What could the issue be? Transmission? Shift box? I plan on changing to a short throw shifter in the future.

- I have a 2006 Honda Metropolitan, WHen cold it starts up but i have to keep revving it to keep it going? I have already replaced the battery, air filter, spark plug, did an oil change, got new gas, cleaned out the carb. Once it is warm i have no issues with it. It usually takes about 3 to 4 min of driving it to get it warm but once warm everything is fine with stopping and going.
