The criteria to replace a rotor is either it is reaching the minimum thickness or warping is present. In spite of the other 'experts' answers it is perfectly possible. The rotors may be poor quality and/or the pads may be too abrasive. I have replaced rotors at 20,000 miles. So make sure the replacements are of a good known make and be sure to replace both sides as this is a safety matter.

Very unlikely, but certainly possible. However, this would generally only be caused if the rotor got overheated frequently or your braking pattern was rough and abusive.

Shouldn't unless it was faulty or you drove down a big mountain braking all the way and overheated the discs so they warped.

If it does, there's something causing it to wear that fast. Either the caliper is sticking or the other brake isn't doing it's job. I'd say there's a warranty problem there.

It should not have to be replaced until the brake pads wear out. If you let the vehicle sit for long periods of time rust can build up and be a problem. Hard braking with too much weight in the vehicle can also warp the rotors.
