Cold means that the engine has not been run for 6-8 hours. The under hood sticker(may be on the hood)will have the clearance specs and if it's set hot or cold.
No. Ambient temp is fine. Reason for this is if carried out on a hot engine. Would be wrong when cool.
Gaps are set to allow foe heat expansion of parts.
Hence exhaust gets hotter so tends to have slightly bigger gap setting point.
